Jay Duty is a seasoned healthcare executive with over 20 years of experience leading growth, transformation, and innovation across post-acute care. Jay brings deep expertise to the American Health at Home Fund in scaling home health and hospice operations, aligning providers with value-based care models, and optimizing organizational performance. Jay is the current Chief Operating Officer of Maxwell Healthcare Associates. Previously, he held senior leadership roles at Enhabit Home Health &amp; Hospice as SVP, Chief Corporate Development Officer, and at Encompass Health as Vice President of Network Strategy. In these roles, he played a critical role in forming strategic partnerships with ACOs, health systems, and payors—driving adoption of alternative payment models and joint venture structures. His ability to align payors, providers, and systems around efficient, patient-centered care continues to influence the industry’s shift toward sustainable, tech-enabled home-based services. Jay’s strategic insight and operational leadership are instrumental in advancing the Fund’s mission to build the largest privately owned home healthcare company in the U.S.
